AGREEMENT TO HOLD HARMLESS WAIVER & ASSUMPTION OF RISK

I understand that attendance in a dog obedience training class is not without risk to myself, members of my family or guests who may attend, or my dog, because some of the dogs to which I will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest amount of care.  I agree to keep my dog under control at all times.  Furthermore, in case of accident, loss or injury I hereby waive and release from any liability the DOG OBEDIENCE GROUP LTD. and all officers, agents, members, instructors and other persons in any way associated with the training program, and the D.O.G. landlord from any and all liability of any nature, for injury or damage in any way resulting from participation in this class, and I expressly assume the risk of such damage or injury while attending any training session, or any function, of the DOG OBEDIENCE GROUP, or while on the training grounds or the surrounding area.  Furthermore, I will not hold the DOG OBEDIENCE GROUP responsible for any actions of my dog after completion of this class. I hereby agree to abide by the training rules and decisions of the DOG OBEDIENCE GROUP.  I understand that I will not bring my dog to training class (but I myself will attend) if the dog is ill, in season, has any communicable canine diseases or condition, OR if any such conditions or diseases exist in any dog in my household. When agility equipment is being used in class or practice, I understand no dog will be allowed on any obstacle without a D.O.G. representative present.  Anyone not following this procedure will be dismissed from the class or practice.  NO HARSH TRAINING METHODS WILL BE TOLERATED.  I understand I MUST supply a copy of my dogs rabies certificate (for dogs over 6 months old) before dogs will be allowed in class!!!  Coupons have no cash value and may not be combined.

Dates:  Classes meet once each                          week

Registration:  In advance only.  Classes are filled on a first-come--first served basis.

Training:  We train you to train your dog.  Our methods are motivational using positive reinforcement (praise/food) for proper behavior.  Punishment for unacceptable behavior is not allowed.  Choke chains and pinch collars are not permitted.  You must work your dog every day approximately 10-15 minutes to get the full benefit of any class.

Handlers:  Must be at least 14 years of age and/or physically capable of handling the dog.

The night of the class:

1.  Don't feed your dog

2.  Bring proof of rabies vaccination

3.. Bring a plain buckle collar

4.  Bring a 4' to 6' leash--no chain or rope

5.  Bring soft treats for your dog

6.  Wear soft soled shoes (no sandals, heels or boots allowed)

7.  Park at the rear of the building

Cancellation Policy:  If the weather is questionable on the night of class for any reason (rain, snow, ice, tornado, flood, etc), or for reasons of national security, please call (816) 228-4567 one hour before class starting time.  If there is no cancellation message we will be having class.

IT IS THE STUDENT'S RESPONSIBILITY TO CHECK IF WE ARE HAVING CLASS!
